I just went through the process of changing the rear wheel, the article by WingAdmin on Replacing the rear wheel was extremely helpful.

Question: I did not have the washer - spacer on my GL1500 SE 1997.

This is the washer/spacer in question (see image)

Does this washer only go through the narrow part of the axle or is it a spacer, and fits over the widest part of the axle, so it basically just takes up the gap between the swing arm and the collar?

It looks like it might fit over the widest part of the axle, but again I want to be sure.
Honda lists the part for about $10.00 is this just a washer?

Yes, it is just a washer and it's inside diameter is sized to fit the axle up to the large diameter end and stop there.

Mike, so it is
A: a washer that will freely rotate around the widest part of the axle, and the only point is to be a spacer between the swing arm and the caliper bracket, basically preventing the caliper bracket from becoming flush with the swing arm.

B: a washer that will only rotate around the narrowest part of the axle, and prevents the widest part of the axle bolt from moving left into the caliper bracket as the axle nut is tightened.

Mike after re-reading your post, I believe B is correct.

In both pictures, according to the factory manual, I tightened the axle nut to 80 foot pounds, then tightened the axle pinch bolt to 23 foot pounds.

B was correct, the steel washer fits only the smaller OD axle and provides a steel face against which the axle shaft's step is pulled .... so as to prevent damage to the softer aluminum material of the caliper bracket.

I can measure one if need be?

Thank you, the explanation of the purpose of the washer is very helpful. I have one that should fit pretty well for now. I already ordered the "Honda" washer, but I don't know when it will be in.
