Connect the headphone speaker thru a 680 ohm resistor and a 1 microfarad capacitor in series to the "+" Speaker output on each channel. "+" terminal of the capacitor toward the "+" terminal of the stereo.
Connect the headphone common connection to bike ground, do not use the "-" Speaker output.
The resistor reduces amplifier output to where the headphone speaker can handle it, the capacitor to block DC from the radio amp.
On Modern stereos both the "+" and "-" speaker outputs are amplifier-driven out of phase for higher output, and you will usually find 6v nominally with respect to ground on both.

The resistor reduces amplifier output to where the headphone speaker can handle it, the capacitor to block DC from the radio amp.

The problem with that is there is often differential between the audio amp rails and common ground, bikes being a very noisy place, electrically speaking. The rails are well-filtered from supply and ground internal to the radio. You could always use a pair of transformers in place of the resistors and capacitors. That would isolate the headphones, adjust the output level, and solve the impedance mismatch.
